The information provided below is the mandatory guideline for any and all "Advisory" and "Fee Disclaimer." They must be given
to each student participating in an American Heart Association (AHA) class.
These forms are the only approved forms accepted.
Advisory
The American Heart Association (AHA) has advised all of their affiliates that all participants of BLS, ACLS and PALS classes
must have an AHA textbook (applicable textbook" before, during and after their prospective class.
Student Fee Disclaimer
The American Heart Association strongly promotes knowledge and proficiency in CPR and AED usage; they have developed instructional
material for this purpose. This material is supplied on a cost basis. Its use in an instructional course does not represent
sponsorship by the American Heart Association. Any fees charged for such a course do not represent income to the American Heart
Association. The American Heart Association does not endorse any program or service other than those presented within American Heart
Association produced materials.
